Paying for your health care

Oak Orchard Health makes it simple for you to pay for your health care. If you have insurance, you must present your current insurance card at the time of your visit. Co-pays and self-payments are due at the time of service. Our sliding fee scale makes sure you can still get the health care you need regardless of income or insurance status. This program offers patients discounts on services based on family size and annual income. Patients approved for the Sliding Fee Program must also pay their co-pay at the time of the visit.

  • Public health insurance (Medicare, Medicaid, Child Health Plus)
  • Cash, check, credit/debit card
  • Sliding fee scale, based on family size and income

We can help you enroll in health insurance coverage. It’s easier than ever, and we have an insurance enroller right here to help you figure things out if you don’t have insurance. Just call (585) 637-3905 and ask for a facilitated enroller at your nearest Oak Orchard Health office or to make an appointment.

*If you select Oak Orchard Health as your medical home, one of our physicians must be listed as your primary care physician with your insurance company. If you do not provide this information to your insurance company, then you will be responsible in full for your bill from Oak Orchard Health.

**The copay for your Vision appointment would be considered a specialist copay and not a primary care physician copay.
